On the home page, click on the Your Lifestyle menu item. From here, click or tap on the Get started button on the Heart Health Lifestyle assessment card to start the assessment.&nbsp;<img class="image_resized" style="width:81.33%;" src="https://cdn-assets.productfruits.com/924e522d-d909-4ac0-bb88-ac5938585499">Simply follow the on-screen instructions to complete the assessment.

After completing your Heart Health Lifestyle Assessment you will be able to download your PDF report. The report download is available from the Assessment card on the Your Lifestyle section in the app. To download the report simply click on the Report download button as per the screenshot below.<img class="image_resized" style="width:81.94%;" src="https://cdn-assets.productfruits.com/18225457-1fc3-4815-a786-d0e07b89e550">
